Noxafil 40mg/mL by Merck Sharp & Dohme is a cherry-flavoured triazole antifungal oral suspension containing posaconazole, used against refractory invasive fungal infections, including aspergillosis, fusariosis, chromoblastomycosis, mycetoma, coccidioidomycosis, and oropharyngeal candidiasis. It works by blocking ergosterol synthesis, destabilising the fungal cell membrane while sparing human cells. Each dose must be taken with food to maximise absorption, and the bottle must be shaken well before measuring with the provided dosing spoon. The oral suspension, tablets, and IV form are not interchangeable and have different dosing.

How It Works
- Posaconazole is a triazole antifungal that targets a fungal cell membrane enzyme called lanosterol 14α-demethylase (CYP51).
- By blocking this enzyme, it prevents fungi from producing ergosterol, an essential component of their cell membrane.
- Without ergosterol, the fungal cell membrane becomes unstable, and the fungus cannot grow or survive.
- Because human cells use cholesterol rather than ergosterol, the antifungal effect is selective to fungi.
Dosage, Uses & Possible Side Effects
Dosage & Administration:
- Treatment of refractory invasive fungal infections: 200mg (5mL) four times daily.
- Oropharyngeal candidiasis: loading dose 100mg twice on day 1, then 100mg once daily for 13 days.
- Each dose should be taken with food or a nutritional supplement to maximise absorption.
- Shake the bottle well before each use; measure carefully with the provided dosing spoon (2.5mL and 5mL markings).
Uses:
- Treatment of refractory invasive aspergillosis in adults intolerant of or unresponsive to amphotericin B or itraconazole.
- Fusariosis in adults refractory to or intolerant of amphotericin B.
- Chromoblastomycosis and mycetoma in adults refractory to or intolerant of itraconazole.
- Coccidioidomycosis in adults refractory to or intolerant of standard therapy; also oropharyngeal candidiasis.
Possible Side Effects:
- Common: nausea, vomiting, diarrhoea, headache, fever, and changes in liver enzymes.
- Serious: hepatotoxicity, QT interval prolongation, and significant drug interactions (especially with sirolimus, which is contraindicated).
- Regular monitoring of liver function and electrolytes (potassium, magnesium) is recommended during treatment.
