Dupixent is a biologic injection that blocks the IL-4 and IL-13 inflammation pathway behind many type 2 allergic conditions. It is FDA-approved for moderate-to-severe atopic dermatitis (from 6 months), eosinophilic or steroid-dependent asthma, nasal polyps, eosinophilic oesophagitis, prurigo nodularis, and eosinophilic COPD. It is given subcutaneously every 2 weeks for most adult uses, and live vaccines should be avoided during treatment.

How It Works
- Dupilumab is a fully human IgG4 monoclonal antibody.
- Binds to the IL-4 receptor alpha (IL-4Rα), blocking signalling of both interleukin-4 and interleukin-13.
- This dampens type 2 inflammation, the central pathway driving atopic dermatitis, eosinophilic asthma, CRSwNP, and EoE.
Dosage, Uses & Possible Side Effects
Dosage & Administration:
- Atopic dermatitis (adults): loading dose 600mg (two 300mg injections at different sites), then 300mg every 2 weeks.
- Asthma (adults/adolescents ≥12 years): 400mg or 600mg loading, then 200mg or 300mg every 2 weeks (depending on indication and steroid dependence).
- CRSwNP: 300mg every 2 weeks.
- Inject subcutaneously into the thigh or abdomen (upper arm if administered by caregiver). Rotate sites.
Uses:
- Moderate-to-severe atopic dermatitis in patients ≥6 months of age inadequately controlled with topical therapies.
- Add-on therapy for chronic rhinosinusitis with nasal polyps (CRSwNP) in adults.
- Eosinophilic oesophagitis (EoE) in patients ≥1 year and weighing ≥15 kg.
- Add-on maintenance for adults with inadequately controlled COPD with eosinophilic phenotype.
Possible Side Effects:
- Common: injection site reactions, conjunctivitis (especially in atopic dermatitis), oral herpes, eosinophilia (usually transient and asymptomatic).
- Serious: hypersensitivity reactions including anaphylaxis and serum sickness (rare), keratitis, eosinophilic conditions (vasculitis or eosinophilic pneumonia, sometimes after oral steroid taper).
- Patients with helminth infections should be treated before initiating dupilumab.
