Nucala is a monoclonal antibody injection given every 4 weeks to control diseases driven by high eosinophil levels. It is approved for severe eosinophilic asthma (from age 6), eosinophilic COPD, chronic rhinosinusitis with nasal polyps, EGPA, and hypereosinophilic syndrome. By blocking IL-5, it reduces eosinophils and the inflammation they cause, helping lower flare-ups and oral steroid use.

How It Works
- Mepolizumab is a humanised IgG1κ monoclonal antibody.
- It binds interleukin-5 (IL-5), preventing IL-5 from interacting with its receptor on eosinophils.
- This reduces production and survival of eosinophils, decreasing airway and tissue inflammation in eosinophil-driven diseases.
Dosage, Uses & Possible Side Effects
Dosage & Administration:
- Severe eosinophilic asthma (adults and ≥12 years): 100mg subcutaneously every 4 weeks.
- Children 6–11 years (asthma): 40mg every 4 weeks.
- EGPA and HES: 300mg every 4 weeks (administered as three 100mg injections at separate sites).
- CRSwNP: 100mg every 4 weeks.
- Inject into the upper arm, thigh, or abdomen.
Uses:
- Add-on maintenance treatment for severe eosinophilic asthma in patients ≥6 years.
- Treatment of adults with eosinophilic granulomatosis with polyangiitis (EGPA).
- Treatment of adults with hypereosinophilic syndrome (HES) for ≥6 months without identifiable non-haematological cause.
- Add-on maintenance treatment of chronic rhinosinusitis with nasal polyps (CRSwNP) inadequately controlled with nasal corticosteroids.
Possible Side Effects:
- Common: headache, injection site reactions, back pain, fatigue, upper respiratory tract infection.
- Serious: hypersensitivity reactions, including angioedema and anaphylaxis (usually within hours), herpes zoster reactivation.
- Less common: parasitic (helminth) infections. Pre-existing infections should be treated before starting.
