How It Works
- Dabigatran etexilate is a prodrug converted to active dabigatran by esterases.

- Inhibits both free and clot-bound thrombin, plus thrombin-induced platelet aggregation.
Dosage, Uses & Possible Side Effects
Dosage & Administration:
- Atrial fibrillation (CrCl >30 mL/min): 110mg orally twice daily.
- DVT/PE treatment or recurrence prevention: 110mg twice daily after ≥5 days of parenteral anticoagulation.
- Swallow capsule whole with water, with or without food; do not open, break, or chew.
- Restart promptly after surgical interruption; reversal agent (idarucizumab) available for emergencies.
Uses:
- Reduces risk of stroke and systemic embolism in patients with non-valvular atrial fibrillation.
- Treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE) in adults previously treated with a parenteral anticoagulant.
- Reduction of recurrent DVT and PE in previously treated adults.
- Prophylaxis of DVT and PE after hip replacement surgery; treatment of VTE in pediatric patients aged 8 to <18 years.
Possible Side Effects:
- Common: gastrointestinal symptoms (dyspepsia, gastritis-like symptoms), bleeding.
- Serious: major bleeding (>15% in some indications), gastrointestinal bleeding.
- Boxed warnings: premature discontinuation increases thrombotic risk; spinal/epidural hematoma with neuraxial anesthesia.
